From daily living support to complex nursing care, delivered by qualified Registered Nurses.
Health Support Care was started by Registered Nurses who wanted to bring genuine clinical care into people’s homes and everyday lives. With over eight years in acute nursing and five years in aged and disability care, we support people across Brisbane, Gold Coast, Ipswich and Caboolture, with the skill of a hospital team and the warmth of someone who truly knows you.
From a hand with everyday tasks to complex clinical care, our nurse-led team is here for you.
Nurse-led support for higher health needs, including feeding tube care, medication management, dysphagia care, airway-related support and post-operative nursing at home.
Practical support for NDIS participants living with disability, shaped around their routine, goals, preferences and day-to-day needs.
Respectful care for older Australians and people living with dementia, with a focus on safety, familiarity, dignity and daily comfort.
Some support needs call for more than a helping hand. They need real clinical expertise. Our nurses specialise in care that many providers can’t offer: feeding tube care (Percutaneous Endoscopic Gastrostomy (PEG), Percutaneous Endoscopic Gastrostomy with Jejunal extension (PEG-J), Nasogastric tube (NGT) and Nasojejunal tube (NJT)), tracheostomy support, ventilator-dependent care, and pre- and post-operative nursing at home. With backgrounds in gastroenterology, endoscopy and dysphagia care, we make complex care feel safe, comfortable and personal.
From daily living support to complex nursing care, Health Support Care brings Registered Nurse experience into the home, with care shaped around the person, their health needs and their goals.
Health Support Care is a registered NDIS provider under the NDIS Quality and Safeguards Commission, and we also deliver Aged Care and DVA services. Whether your plan is agency managed, plan managed or self managed, we’re here for you. And if you’re new to the NDIS, don’t worry: we’re always happy to sit down and help you understand what’s possible.
Whether you’re a participant, a family member or a support coordinator, we’re happy to talk through how we can help. Call, email or send us a message and we’ll get back to you as soon as we can.
We welcome referrals from support coordinators, plan managers, hospitals and families. Send through the referral form and our nurse-led team will be in touch to discuss the participant’s support needs.
